Pc Wizkid & Programmer :)
Uninstall usb drivers from device manager, reboot and let it find the drivers again... if that dont solve it then chances are its dead.
also try in safemode
good luck

It Might be due to ,
- Host controller hardware malfunction
- Incorrect USB driver versions
- No support for USB 2.0
- Additional system hardware or configurations that may change the timing
- Attached hubs
- Attached devices
Try to perform these actions ,
Check for any loose USB connections . Re-plug the USB device tightly and firmly into the USB port.
From the Start menu, select the Run -> type in Devmgmt.msc and then click Ok. From the Action menu, click Scan for hardware changes.
Click Start -> Run. Type devmgmt.msc , and then click Ok. Expand the Universal Serial Bus Controllers' node
- Right-click the USB Root Hub and then click Properties.
- Click the Power Management tab.
- Click to clear the Allow the computer to turn off this device to save power check box, and then click Ok.

Edit*
While Surfing Net , I came across this solution . Just try this . Not sure this is 100% correct .
The driver or inf file is damaged/missing. Visit another computer with win xp, search in windows map for files named usbstor. Copy files usbstor.inf and usbstor.pnf into windows/inf folder and usbstor.sys into windows/system32/drivers. Replace if asked. Now just search for new driver in device manager and thats it, usb storage will be recognized.

i know of one trick you can do to tell u one way or another if its dead or not, insert a bootable usb drive "or stick" ,Go into Bios and set it to boot from usb first..
then attempt to boot from the usb stick
, if it boots then u know its windows!
just one damaged usb port can stop all the ports functioning due to a short
